The Care Coordination Assistant works as part of a team assisting the Care Coordinator with administrative tasks such as completing required documentation and forms, coordinating referrals, and obtaining necessary resources. This role requires excellent collaboration, organization, strong customer services skills, and the ability to work independently. This position offers the opportunity to work from home with 10% local travel required.
