Licensed Practical Nurse (LPN) Waukesha, West Allis, New Berlin

Position Overview:

The Clinical Assistant works alongside our provider team traveling to local Assisted Living and Memory Care Units where direct patient care is provided. Duties include medication reconciliation, pre-rounding, rounding with providers while updating EMR, obtaining vitals and concerns, and assisting in provider paperwork. The role requires excellent organizational and customer service abilities. 

  • Assists providers and facilitates an efficient, organized clinical flow
  • Interact with patients for annual wellness visits and document as necessary
  • Ensures electronic medical records are accurately updated on a daily basis
  • Sets up e-prescriptions under direct supervision of providers
  • Medication reconciliation
  • Works independently and with provider teams, internal staff, and facility staff
  • Assists team and internal staff with implementation of new quality measures, policies, and procedures
  • Taking notes and documenting observations alongside provider during patient visits
  • Monitoring communication portal messages and summarizing for provider as necessary

Qualifications:

  • Licensed Practice Nurse
  • Comfortable navigating electronic medical record systems and working with patient care notes
  • Knowledge of medical coding a plus
  • Excellent interpersonal and customer service skills
  • Detail oriented and accurate
  • Ability to problem solve and work well under pressure
  • Computer proficient
  • 55+ WPM typing, medical terminology knowledge and understanding of patient care notes
  • Demonstrated compatibility with Bluestone’s mission and operating philosophies
  • Demonstrated ability to read, write, speak, and understand the English language
