Overview

Join our growing practice as a full-time Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients living with a disability residing within Group Homes and Facilities in and Minneapolis/St. Paul area.

Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive, and includes all members of the care team, patients and their families. Our providers have deep knowledge of the unique aspects of Group Home settings and of patients who have intellectual and physicial disabilities.

Position Description

  • Provide on-site, team-oriented primary care to patients within the group home setting living communities
  • Part-time providers care for patients weekdays; tailored to part-time desires and needs
  • Manage a panel of patients autonomously seeing approximately 8-14 patients per/day
  • Ability to design and manage your schedule 
  • Develop and implement care plans appropriate to patients and their desires
  • Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers 
  • Collaborate with community partners within home care, hospice, and other resources as needed

 
Qualifications

  • Graduate of NP program or PA program from an accredited school
  • Must have and maintain a valid Minnesota License
  • Professional clinical experience in primary care
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required
  • Demonstrated compatibility with Bluestone’s mission and practiticing philosophies
  • Travel throughout Twin Cities Metro required

Preferred Qualifications

  • 2+ years of clinical experience and/or group home experience in another role
