Role Overview:
Transform care for complex, chronically ill patients in assisted living and memory care settings. We’re seeking a dynamic Physician Team Leader (MD or DO) to provide direct patient care while serving as a clinical leader and collaborator. In this essential role, you’ll partner with nurse practitioners, physician assistants, and an integrated care team to expand our innovative care model, improve patient outcomes, and ensure high standards of quality through mentorship, clinical guidance, and open team communication.

 

Key Responsibilities:

  • Provide mentorship, clinical guidance, and quality assurance for a team of up to 8 APPs
  • Lead 1:1 and team meetings to support performance, clinical excellence, and standardization of care
  • Serve as Primary Care Provider for a small panel of patients or co-manage a patient panel alongside an APP.
  • Participate in case reviews, practice improvement discussions, and regular case rounds
  • Sign regulatory documents and support telehealth visits or chart reviews for complex cases
  • Collaborate with Clinical Practice Directors on care initiatives and implementation
  • Support regulatory compliance in line with CMS and state requirements
 

Qualifications:

  • Graduate of an accredited ACGME/AOA residency training program
  • Board Certified or board prepared with the American Board of Medical Specialties or American Osteopathic Association
  • Licensed or ability to practice within the state of Florida
  • Ability to maintain DEA certification for the State of Florida
  • Experience in geriatrics, primary care, or complex patient management preferred
  • Strong communication, leadership, and team-building skills
