Practice Development Lead

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview:
The ideal Practice Development Lead is a strong communicator and relationship-builder who thrives in both operational detail and strategic partnership management. They are organized, proactive, and confident in representing the practice in internal and external settings.
Schedule: Full time position. Successful candidate will be traveling in the field 30% of the time and will work remote (work from home) 70% of the time.
Location: Minneapolis/St.Paul, MN & surrounding suburbs.
Salary: $68,000 - $70,000 annually. Salary will be commensurate with experience.
Responsibilities:
Provider Team Operations
- Oversee scheduling, workflow, and coordination for assigned provider team(s) to ensure efficient practices and team resources.
- Serve as liaison between clinical teams, administrative support, and leadership to resolve operational challenges.
- Anticipate provider team needs and proactively remove barriers to care delivery.
Corporate & External Relations
- Build, manage, and grow relationships with key corporate partners, ensuring active engagement, service excellence, and long-term collaboration.
- Act as primary point of contact for assigned communities & corporate partners, managing escalations, performance reviews, and strategic planning discussions.
- Represent the practice with referral partners, healthcare networks, and community organizations to expand visibility and growth opportunities.
Strategic Growth & Development
- Collaborate with leadership to align provider team operations with business development objectives.
- Identify opportunities to expand program, strengthen referral pipelines, and improve partner satisfaction.
- Monitor and analyze key performance indicators (provider utilization, referral activity, partner satisfaction) to provide leadership with actionable insights.
Qualifications:
Education/Certification/Experience
- Bachelor’s degree in healthcare administration, business, or related field preferred; equivalent experience considered.
- Experience in healthcare operations, practice management, or business development required.
Knowledge/Skills/Abilities
- Strong organizational, communication, and problem-solving skills.
- Ability to balance detail-oriented operational tasks with strategic relationship management.
- Proven ability to build trust and credibility with providers, staff, and external partners.
