Primary Care Practice Support Specialist

Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview:
This role is responsible for the day-to-day management and coordination of critical practice operations and reports to the Senior Business Director of Florida.

Schedule: Full-Time position with regular business hours — Monday thru Friday
Position work in office 1 to 3 days per week with remaining days as work-from-home/administrative days
Salary: $75,000 - $85,000 annually. Salary will be commensurate with experience.
Responsibilities:
Provider Resourcing
- Tracking assignment of providers to communities
- Presenting Senior Business Director with resourcing plans
- Supporting Senior Business Director and Clinical Practice Director with provider communication
Provider Scheduling
- Ensure provider scheduling standards are upheld and schedules are complete
- Support teams with scheduling strategies as needed
Provider Onboarding
- Scheduling of all new provider meetings and activities included in provider onboarding process
- Community based schedule for first 9 months of practice - ensure collaboration with Clinical Practice Director
Data Review (15%): Understand key primary care metrics, including fee-for-service and value based care, and perform weekly gap analysis with recommendations
Administrative Support (15%): Manage key tasks and administrative workflows for the Senior Business Director or Clinical Practice Director
- Track agenda topics and take meeting notes
- Support Senior Business Director with tracking and coordination of internal and external meetings and relationships
Required Education/Certification/Experience
- Bachelor’s degree preferred - in business, operations, or a related field.
- 3+ years of experience in project management, program management, or an operational role within a relevant industry.
Knowledge/Skills/Abilities
- Proven ability to manage multiple competing priorities simultaneously, exceptional organizational skills, and strong written and verbal communication.
- Proficiency in the entire Google Workspace/Google Suite.
- Demonstrated ability to work with and synthesize data from various sources to produce accurate, weekly and monthly KPI reports.

- Detail-Oriented: Has a meticulous approach to data integrity, reporting, and budget tracking.
